Differential Diagnosis + MSE Walkthrough With Vignettes

Differential diagnosis plus the Mental Status Exam is the highest-value skill on the ASWB Clinical exam. Nearly every Assessment and Planning vignette hides the answer in three clues: trigger, timeline, and core feature. Learn the walkthrough below, then prove it on an ASWB Clinical practice test.

The 4-Step Walkthrough

  1. Trigger: what started it — death, trauma, substance, medical change, or nothing identifiable.
  2. Timeline: 2 weeks (MDD), 3-month stressor link (adjustment), 12-month grief (prolonged grief), 1-month post-trauma minimum (PTSD).
  3. Core feature: anhedonia (MDD), longing for the deceased (grief), intrusions plus avoidance (PTSD), worry across domains (GAD).
  4. Rule-outs: substance, medication, medical condition, then culture and environment before finalizing.

Highest-Yield Differentials Table

PairDeciding cluePick this when
MDD vs prolonged griefCenter of painLonging for deceased + 12 months = grief; pervasive anhedonia + worthlessness = MDD
PTSD vs adjustmentTrauma criterion + intrusionsLife-threat trauma with flashbacks and avoidance = PTSD; non-Criterion-A stressor = adjustment
Bipolar I vs MDDHistory of maniaOne manic episode ever = bipolar I, even if current episode is depressive
Schizophrenia vs psychotic depressionMood-psychosis timingPsychosis only during mood episodes = mood disorder with psychotic features
Alcohol use vs alcohol-induced disorderTiming of symptomsDepression only during heavy use and withdrawal = substance-induced until proven otherwise

MSE: What Changes the Answer

The MSE is not vocabulary trivia — each element re-routes risk and diagnosis:

  • Appearance, behavior, psychomotor: agitation or retardation corroborates severity; bizarre behavior pushes toward psychosis workup.
  • Speech and thought process vs content: process is how thinking flows (tangential, flight of ideas); content is what fills it (delusions, SI/HI, obsessions). Flight of ideas signals mania; thought blocking with paranoia signals psychosis.
  • Mood vs affect: mood is reported, affect is observed. Constricted affect with flat delivery plus anhedonia strengthens MDD.
  • Cognition: disorientation or memory gaps in an older adult means medical workup before any dementia label.
  • Insight vs judgment: poor insight with good judgment can still engage; poor judgment with plan, means, and intent means act now.

Risk Wording That Decides Disposition

PresentationDisposition
Ideation + plan + means + intentImmediate protection: stay, mobilize, hospitalize, document
Ideation without plan or intent, protective factors presentSafety plan, means restriction, collateral, close follow-up
NSSI without intent to dieAssess function of behavior, teach DBT distress-tolerance skills, monitor — not involuntary hold by itself
Command hallucinations to harm othersDuty-to-protect analysis: identify victim, assess credibility, warn and protect per law

Final drill: for every vignette you miss, write trigger, timeline, core feature, and the one MSE word that should have tipped you. Ten cards like that beat ten hours of rereading.
